OJT - Record Book (Blue Book)

1Who do I contact if I have questions about my OJT Record Book or need a report of my employer hours?

2When should I submit my OJT Record Book?

You must submit your OJT Record Book every time you reach 1,000 work hours. Your book must be signed by your job supervisor.

3How can I submit my OJT Record Book?

You can submit your completed OJT Record Book to our office at: 3033 Alvarado St., San Leandro, CA 94577

You may:

Mail it to our office.
Submit it in person during office hours (8am-4pm, Monday - Friday).
Submit it in person after office hours by dropping it in the silver mailbox located in front of the building.

Apprentice Classes

1What should I do if I cannot attend my scheduled class?

Use the Contact Us Form as soon as possible to explain your situation and request rescheduling (based on availability).

Please Note: Documentation is required for any absence to be considered EXCUSED.

Absence due to illness – Please provide a doctor’s note or positive COVID-19 test result.
Absence due to accident – Please provide a tow receipt, police report, or similar documentation.
Absence due to travel plans – Please provide an Airline ticket or hotel booking confirmation.
Absence due to Wedding or Funeral – Please provide documentation showing service dates.
Absence due to childcare issues – Please provide a letter from your child’s daycare or a signed handwritten note from the caretaker stating the dates care was unavailable.

Withdrawing from the Apprenticeship Program

1What should I do if I no longer wish to be an Apprentice?

If you decide to resign from the program, you must contact the Training Coordinator and submit a written resignation letter. Your resignation is subject to review and approval by the Sound and Communication Sub-JATC Committee.
